We are thrilled to announce that Consumer Direct Care Network South Dakota (CDSD) is the new Agency with Choice (Family Support 360) provider for the South Dakota Division of Developmental Disabilities (DDD).
CDSD is excited to support South Dakota’s individuals and families with personalized, self-directed care and we are committed to providing excellent customer service and support across the state. Over the years, we have empowered thousands to manage their own care, and stay in their homes and communities.
If you receive services through the Agency with Choice program (Family Support 360), please check our website frequently for updates. In the coming weeks, we will also email you additional information on the new changes.
